Prepare for your Visit

Before your first appointment at Living Link Massage Therapy, please do your best to include the following:

  • Arrive 15 minutes before the start of your appointment.  This allows time for you to settle in, for us to get to know each other a little, and for an intake process before we begin your session on time.
  • Choose 1 of 3 options for filling out my required intake form
    1. Fill out online intake form before coming in to your session (form and site are secure and HIPAA compliant).
    2. Print and fill out paper intake form at home.
    3. Fill out paper home in-office during the intake process before our massage.
  • Avoid any large meals right before your massage.
  • Bring a copy of any relevant scan/test results, if you have them.

Before a regular appointment, please do your best to include the following:

  • Arrive 5-10 minutes before the start of your appointment.  This allows time for you to settle in, update me on how you are doing and any issues that have come up, as well as your goal for the massage before we begin your session on time.
  • Avoid any large meals right before your massage.

Office Policies

Cancellations and missed appointments

Please provide 24-hour notice of cancellation prior to your scheduled appointment. Anything less may be charged accordingly. I require this time to reschedule someone else in your time slot.

If you miss an appointment, all parties suffer: you miss out on treatment, I now have an open block of time, and the person that couldn’t get in during your scheduled time missed an opportunity. My policy is intended to respect the time constraints and integrity of all parties.

Reasons for being dismissed/denied treatment: Patients who show inappropriate conduct or whose conduct raises safety concerns may be denied treatment.

Financial Policy

Your payment is due in full at the time of service.  For your convenience, I accept cash, check or credit cards, PayPal and Venmo.

Insurance Policy

At this time, I do NOT accept health insurance and am not in-network with any carriers.

If you would like to investigate whether your carrier will cover massage, I can supply you with the necessary forms to access all the information you need to make an informed choice (see below).

If you know that your carrier covers massage, and you have a prescription from a doctor (that includes all the necessary components), I will be glad to bill your insurance.

If your insurance company does not cover massage, I will gladly give you a receipt for all of your treatments so you can submit them to your insurance company for reimbursement.

Online Forms

If you would rather not use the online format, you can also print and completely fill out the Living Link Massage Therapy Intake Form before your first session and bring it with you.

If you would like me to try and bill insurance on your behalf, download this Major Medical Coverage Verification Form and use it to gather all necessary information when contacting your carrier.

In order for me to bill insurance, a physician’s prescription is necessary.  Have your physician either fill out this Massage Therapy Prescription, or make sure all details shown on the form are included on the physician’s prescription that you receive.
